Explain your rationale.According to ProCFO (2022), transition management is a process of getting the best out of change and managing the transition from one state to another. It includes defining the transition, sustaining the change, identifying the set of activities that are undertaken to ensure a smooth and successful transition, and supporting the people involved and impacted by the initiative. Staff must be included in all the phases of transition management for them to feel comfortable about the change and to encourage them to work towards the goal. The successful transition depends on the interaction between the individuals who are leading the change and those impacted by the change initiative. To encourage buy-in, it is necessary to clarify the need for change, what the organization wants to achieve, how they will get there, what successful implementation looks like, and how it will impact staff, departments, and the organization moving forward, outlining the strategies and creating milestones that should be communicated effectively, and describing the roles and responsibilities of staff in the transition including the expected deliverables and aligning them (ProCFO, 2022).The Electronic Health Record system has transformed the collection and standardization of patient information. Providers now have patient records that are accessible and readily available, making care more efficient and accurate. Healthcare reforms have made it possible for EHR to be accessible across the entire care community by connecting all systems and delivering a single view of a patient’s information (Mushtaq, 2015). In 2011, the Centers for Medicare and Medicaid Services [CMS] established the Medicare and Medicaid Electronic Health Record (EHR) Incentive Programs to encourage eligible providers, hospitals, and critical access hospitals to adopt, implement, upgrade, and demonstrate meaningful use of EHRs to qualify for incentive payments through the program (CMS.gov, n.d.). To qualify for the meaningful use incentive program, providers and hospitals must meet the meaningful use requirements that intend to improve quality, safety, and efficiency, reduce health disparities, engage patients and their families, improve care coordination, and maintain the privacy and security of patient health information (Mushtaq, 2015). According to Mushtaq (2015), the proposed rule’s main objective is to improve the accessibility and exchange of data that serves several types of programs and reduces the required reporting, enabling providers to focus on objectives that support the advanced use of EHR technology and quality improvement, including health information exchange. The benefits of EHR through meaningful use include providing clinically relevant data at the point of care, improving patient compliance to improve outcomes, and providing access to data to drive improvement and change behavior. Implementing changes in the organization to meet meaningful use objectives, means higher quality data that is accessible to all, encourages providers to provide better care, increases patient satisfaction and engagement through active participation in their care, and supports a more efficient program (Mushtaq, 2015).
