worked. Raised by mother, she never knew her father. Mother with hx of generalized anxiety and
was verbally abusive, abused benzodiazepines; no substance hx for patient. No previous
psychiatric treatment. Has one glass red wine with dinner. Sleeps 12-13 hrs.; appetite decreased.
Has overactive bladder, untreated. Allergic to Zofran; complains of headaches, takes prn
Tylenol, has diarrhea 2-3 times weekly, takes OTC Imodium.
Mrs. weirder is in the office because she wanted to talk. She feels like she stuck in the house. She only go out Tuesdays and Saturday when her husband is at home. When he is home he go out with her. She go out to walk. She walk only go to the end of the block, and then I cross the street, and turn around, and I go back around the cul-de-sac. I’ll do that three times. When she is around people she get shortness of breath, and everything in the world is so close to her , everything she feels tight,air in the body and chest and she cant express what is wrong. She is afraid to die. Other things are like when car goes fast. She has flashes of murderess and rapes that seen on the news. She feels that its close. So she feels safte to stay in home. She does not know how long she is feeling like that . Last time she spent tome outs ode was fifteen years ago. She is afraid of dying. Her mother passed away she is sacred to drive in the free way. She has to have extra courage to come today and she is here because she have a new grandson and her daughter is not palnning to come right away
