He noticed yesterday that there was some cloudy, beige discharge coming from the abdominal suture line and he felt ‘hot’. He called the surgeon who operated on his bullet wound and the surgeon has told Mr. Young to go to the ED to be assessed. At the ED at 12:00, his VS were BP 130/82, Pulse 90, Respiratory rate 18, and Temp 101.6 Orally. O2sat 97% on room air. Mr. Young was also complaining of pain in his abdomen of a “5”. A wound culture was taken, and labs were drawn. The nurse observed that the suture line was intact and that the drainage was cloudy and beige. A peripheral IV site in his left forearm with a #20-gauge catheter was inserted and is clean, dry, intact (CDI) and patent. An IV of Normal Saline at 100 mls/hr (calculate gtts/min) was started and a piggyback medication of Unasyn 2 grams in 100 mLs of NS (1st dose) was also hung to infuse over 30 minutes (calculate gtts/min) – both administered at 14:00. Tylenol 650 mg po PRN was also administered at 14:00. Mr. Young voided 300 mLs in the urinal of clear, yellow urine. He drank 240 mLs of water with the Tylenol. Labs have just been posted and need to be reviewed and any abnormalities reported to the surgical team. The patient will also have a PICC line inserted in the morning to continue the antibiotic at home for several weeks. Education was provided on why a PICC line is needed and the care at home. Visiting nurse consult ordered. Mr. Young stated, “I think I can do this”. 1. In the MaryMount EHR document in the following tabs: Vital Signs, I+O (document the I+O at 16:00 and include the mLs of the IV’s as well as the other Intake and Output), GU, Wounds and Drains, IVs and Lines, and sign off on all the medications in the MAR. calculate the rate of the 2 IV’s infused by gravity with a DF of 10 – record in the IV’s and Lines tab. 2. Write a note in the Nurses Notes to include: a. Date, time (16:00), and signature b. Information about which labs were reported to the surgical team – be specific and no more than 1- 2 sentences.
