Acute Infectious Diarrhea

Mrs. Hannah Cohen is a 98-year-old woman who was admitted to the medical unit on March 2, 2023 for treatment of infectious diarrhea. Mrs. Cohen is a retired school teacher and has lived in Golden Acres long term care facility for the past 2 years since her husband Isaac passed away. She has one son, named Jacob who is married and lives about 10 Km away but visits her frequently at the nursing home. She is of Jewish faith and speaks Hebrew and English fluently. She loves to read and wears reading glasses.

Mrs. Cohen has a past medical history of hypertension, osteoarthritis and osteoporosis. Her only past surgical history is a cholecystectomy at age 53 yrs. Mrs. Cohen has no known allergies and is a DNR status. She takes Bisoprolol 5mg, PO daily, Alendronate 70mg, PO once a week, and Tylenol, 1g, PO q6h PRN for pain. For the past 4 days she has also been taking Loperamide 2 mg, PO, with every episode of diarrhea.

You are the nurse on the surgical unit when Mrs. Cohen and her son arrive.

On assessment her vital signs are Temperature – 37.9 degrees Celsius, Pulse (radial) – 102 bpm, Respirations – 24 breaths/min, Blood Pressure – 148/98 mmHg, O2 sat – 93% on room air. She currently denies any pain but states she often has chronic pain which stops her from walking far. She uses a 2 wheeled walker for mobility and only requires minimal assistance with ADLs. Mrs. Cohen states she sometimes has a problem sleeping due to pain but if she takes Tylenol it helps. Her chest is clear and she is usually continent of urine and feces, though has had some accidents since she became sick.

Mrs. Cohen states she has lost weight since she has been sick as she has no appetite. She states she was around 145 lbs a month ago and now is only 135 lbs. She is 5 feet 6 inches and normally eats a regular Kosher diet. Over the past few days she has eaten very little and reports having 6 episodes of diarrhea in the past 24 hours. Her abdomen is soft and non-distended with hyperactive bowel sounds.

Her skin is intact, but her skin turgor is poor and her mucous membranes are dry and sticky. Her eyes look sunken and she reports feeling tired and lethargic. She has only voided urine once in the past 24 hrs. Currently she has an IV of Ringer’s lactate at 80 ml/hr in her left hand.

Mrs. Cohen describes herself as friendly, optimistic person and says she enjoys many of the social activities in the nursing home. She sometimes feels sad due to the loss of her husband but does not feel she is depressed.

Her bloodwork showed a Na – 148 mmol/L, creatinine – 1.08 mg/dl, serum urea – 9 mmol/L, urine specific gravity – 1.025, her stool sample showed Cl. Difficile.
